refactor(cmd): consolidate remaining styles in ai, php, setup

- ai: Replace task styles with shared TitleStyle, ValueStyle, AccentLabelStyle
- php: Use shared.StageStyle for QA stage headers
- setup: Use shared.TitleStyle instead of hardcoded colour
- shared: Add AccentLabelStyle and StageStyle
