Issue

Template changes affect existing plan references without versioning.

Expected Behavior

Track template versions so plans reference specific versions.

Acceptance Criteria

  • Add version tracking to templates
  • Plans reference template version, not template
  • Allow template updates without breaking existing plans
  • Add migration for version column
  • Add tests for version management
  • Update PlanTemplateService
