Lint

Run linter and optionally fix issues.

Usage

/qa:lint            # Run lint, report issues
/qa:lint --check    # Check only, no fixes
/qa:lint --fix      # Auto-fix where possible

# PHPStan doesn't auto-fix, but can suggest fixes
